The Niagara Rapids Volleyball Club is committed to ensuring each athlete is given the opportunity to participate in team sport and invest in their own personal development in volleyball.

The financial success of the Club is based on full participation by all athletes. By completing this form, I am acknowledging that I agree to the following payment terms:
High Performance Athlete registration fee will be $2150 (12 athletes) – $2550.00 (10 athletes)

Full Club Payment due September 20th, 2024

Half Club Payment due September 20th, 2024
Half Club Payment due November 1st, 2024

❐ Option 3
$900.00 due September 20th, 2024
$900.00 due November 1st, 2024
$remainder due December 1st, 2024

If I do not adhere to this payment schedule, I understand that the athlete will be suspended from further participation in club activities.
